Completing Form I-9 and Uploading Documents

Overview

This guide provides a step-by-step process for completing Form I-9 and uploading necessary documents, ensuring compliance with employment verification requirements. 

By following this guide, individuals can efficiently complete their I-9 forms, saving time and reducing the risk of mistakes that could delay their employment.

  1. Navigate to Adaptive Employee App



  1. Click on the menu icon.



  1. Click on the "Personal" icon.



  1. Click "Form I-9"


  1. Click "FORM I-9"



Entering Information

  1. Enter your legal last name, first name, and middle initial (if any)



  1. Enter your Date of Birth and SSN



  1. Enter your full address



  1. Click "Next"



  1. Select whether or not you used a preparer to help you fill out the form and click "Next"

    Note: if you select "I had assistance from a preparer or translator" they will have to add their information into the form.



  1. Select your citizenship status



  1. Type in your name in the "Signature of Employee" field.




Alert

Important! Your name has to be spelled exactly the same as it was entered on the form earlier, and it has to be in First M Last order.
Make sure there are no extra spaces, commas, or periods or you will get the following error:
"The signature text must be in the format 'First Name' 'Middle Initial' 'Last Name'."




  1. Click "sign"



  1. Click "ACKNOWLEDGE"


Document Verification Requirements

The Form I-9 document process requires confirmation of two distinct elements for every employee:

  1. Identity: Proof that the individual presenting for work is the person they claim to be.

  2. Employment Authorization: Proof that the individual is authorized to work in the United States.

Documents used for verification are categorized into three lists:

List A

Both Identity AND Employment Authorization

U.S. Passport or U.S. Passport Card, Foreign passport with temporary I-551 stamp or printed I-94 indicating asylum/refugee status, Permanent Resident Card (Form I-551)

List B

Identity ONLY

Driver's license or ID card issued by a state or outlying possession of the United States, School ID card with a photograph, Voter's registration card, U.S. military card or draft record

List C

Employment Authorization ONLY

U.S. Social Security Card (unrestricted), Certification of Birth Abroad (Form FS-545 or DS-1350), Original or certified copy of a birth certificate bearing an official seal


Employees must present either one document from List A OR one document from List B AND one document from List C.

To provide a copy of your documents, follow the instructions below:

  1. Click "Upload document"



  1. Click "Choose file" and upload the file



  1. Click "Next"



  1. When done adding all documents click "Complete"



  1. Click "YES, I'M DONE"



  1. Click "Close"
